Assumption of Risk: I understand that participating in basketball activities involves certain inherent risks, including but not limited to, physical injury, property damage, or death. I voluntarily assume all risks associated with my participation in the basketball camp.

Release of Liability: I hereby release, waive, discharge, and covenant not to sue Taylor Middle School, Desert Sun Elite, their respective coaches, staff, employees, agents, and volunteers (hereinafter referred to as "Released Parties") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including but not limited to personal injury, disability, death, property damage, or loss, that may be sustained by me or to any property belonging to me, whether caused by the negligence of the Released Parties or otherwise, while participating in the basketball camp or any related activities.

Indemnification: I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all liability, claims, demands, actions, damages, costs, and expenses (including but not limited to attorney's fees) arising out of or related to my participation in the basketball camp

Photography and Video Release: I grant Taylor Middle School and Desert Sun Elite the irrevocable right and permission to photograph, film, or otherwise record my likeness and voice in connection with my participation in the basketball camp, and to use such photographs, videos, or recordings for promotional purposes without compensation to me.

Emergency Medical Treatment: In the event of any injury or illness that may occur during my participation in the basketball camp, I authorize the Released Parties to obtain emergency medical treatment for me, including but not limited to first aid, CPR, and transportation to a medical facility, as deemed necessary by the Released Parties.

Severability: If any provision of this liability waiver is held to be invalid, illegal, or unenforceable, the remaining provisions shall continue in full force and effect.

Governing Law: This liability waiver shall be governed by and construed in accordance with the laws of New Mexcio, without giving effect to any choice of law or conflict of law provisions.

I have read this liability waiver carefully and fully understand its contents. I am aware that by signing this document, I am waiving certain legal rights, including the right to sue. I voluntarily agree to its terms.
